Muchas personas consultan cuando ya hay dolor persistente, endurecimientos, cambios en la piel o síntomas que parecen ajenos al procedimiento realizado meses o incluso años atrás. El desafío es que estas complicaciones pueden aparecer de forma silenciosa, avanzar por etapas y confundirse con alergias, infecciones comunes o dolores musculares.

    El cirujano plástico Juan Carlos Torres del Río, especialista en cirugía plástica y reconstructiva, con experiencia en retiro de biopolímeros y reconstrucción glútea/tejidos, advierte que la clave para reducir daños es reconocer a tiempo las “señales de alarma” que pueden indicar migración del material o una reacción inflamatoria e inmunológica sostenida. “El error más frecuente es esperar a que el cuerpo ‘se acostumbre’. Cuando hay señales de alarma, el organismo ya está diciendo que algo no está bien. La pregunta no es si ‘se pasa solo’, sino qué está ocurriendo y cómo se detiene a tiempo”, señala.

    Esta alerta no pretende generar pánico, sino orientación: identificar síntomas que deben motivar una valoración médica oportuna, comprender por qué aparecen, y conocer los riesgos de tratamientos improvisados que, lejos de resolver, suelen agravar el cuadro.

    Biopolímeros: por qué pueden complicarse con el tiempo

    En el lenguaje cotidiano, muchas víctimas usan “biopolímeros” para describir sustancias inyectadas con fines de aumento o relleno. El problema es que, en numerosos casos, no se trata de productos médicos aprobados para ese uso, sino de materiales de origen y composición incierta. Incluso cuando el paciente cree haber recibido “algo seguro”, con el tiempo pueden ocurrir reacciones adversas, especialmente si el material se desplaza, se encapsula o activa una respuesta inflamatoria.

    “Hay pacientes que estuvieron ‘bien’ durante años y, de repente, empiezan con dolor, ardor, inflamación, durezas o síntomas sistémicos. Eso no significa que el problema sea nuevo; a veces significa que el cuerpo ya no tolera la carga inflamatoria o que el material cambió de posición”, explica Torres del Río.

    La migración es uno de los fenómenos más reportados por pacientes: el material se desplaza por planos de tejido, a favor de la gravedad, la presión, el movimiento o la manipulación. La reacción inflamatoria también puede activarse o reactivarse, generando episodios agudos o crónicos.

    “Señales de alarma”: síntomas locales que no deben ignorarse

    Aunque cada caso es distinto, el Dr. Juan Carlos Torres del Río organiza las señales de alarma en grupos. La primera categoría es la de síntomas locales (en la zona donde se aplicó el material o en áreas cercanas), que suelen ser los más evidentes.

    1) Dolor persistente, punzante o que empeora con el tiempo

    Un dolor que aparece de forma repetida, que se vuelve más intenso, que impide sentarse, caminar o dormir, o que empeora con la presión, puede indicar inflamación, compromiso de tejidos o afectación nerviosa. No es lo mismo una molestia transitoria que un dolor que progresa.

    “Cuando el dolor se hace parte de la rutina, ya no estamos hablando de un ‘efecto secundario’ normal. Es una señal de que algo está irritando o dañando tejidos”, advierte.

    2) Endurecimientos, nódulos, “bolas” o zonas con textura irregular

    Palpar masas duras, grumos o áreas rígidas puede reflejar encapsulamiento, fibrosis, granulomas o acumulaciones del material. También puede ocurrir que la zona se sienta “acartonada” o con pérdida de elasticidad.

    “Las durezas no son un detalle estético: muchas veces son el lenguaje del cuerpo para decir que está intentando aislar un material extraño. Ese intento puede terminar afectando circulación y tejidos”, explica.

    3) Inflamación recurrente, enrojecimiento o calor local

    Episodios repetidos de inflamación, calor, enrojecimiento o aumento de volumen pueden indicar reacción inflamatoria activa o infección. Si se asocia a fiebre o malestar general, la urgencia aumenta.

    “No todo enrojecimiento es infección, pero toda inflamación persistente merece valoración. Hay pacientes que se automedican con antiinflamatorios y enmascaran un problema que sigue avanzando”, señala.

    4) Cambios en la piel: coloración, manchas, brillo anormal o piel adelgazada

    La piel puede volverse más tensa, brillante, adelgazada o presentar cambios de color (zonas más oscuras o violáceas). Estos cambios pueden relacionarse con inflamación crónica, alteración de la microcirculación o presión del material sobre tejidos.

    “Cuando la piel cambia, ya estamos viendo el efecto por fuera de lo que está pasando por dentro. Es una señal para actuar temprano”, enfatiza.

    5) Secreción, fístulas, heridas que no cierran o salida de material

    La aparición de secreción, drenaje persistente, mal olor, heridas que no cicatrizan o salida de material por la piel es un signo serio. Puede relacionarse con infección, necrosis o trayectos fistulosos.

    “Una fístula no es una ‘heridita’. Es una comunicación anormal que indica daño interno. Requiere manejo médico, no remedios caseros”, advierte.

    6) Asimetrías nuevas o cambios bruscos de forma

    Un glúteo que cambia de forma repentinamente, aparición de abultamientos en zonas inesperadas, “hundimientos”, deformidades progresivas o pérdida de contorno puede asociarse a migración, inflamación, fibrosis o destrucción de tejido.

    “La asimetría que aparece después de años y progresa suele tener una causa. Y esa causa muchas veces no es ‘músculo’, es material desplazado o tejido afectado”, explica.

    Señales de alarma de migración: cuando el problema “se mueve”

    La migración puede manifestarse con síntomas en áreas diferentes a donde se aplicó el material inicialmente. Esto confunde al paciente: cree que el dolor en la espalda baja, en muslos o en ingle “no tiene relación” con un procedimiento de glúteos, por ejemplo.

    7) Dolor o endurecimiento que aparece en muslos, caderas, ingles o región lumbar

    Cuando el material se desplaza, el paciente puede notar presión, nódulos o dolor en rutas de migración. En glúteos, se reporta con frecuencia sensación de peso o “bajamiento” hacia el muslo.

    “Si el dolor ‘se corre’ o aparecen bolitas lejos del punto de aplicación, hay que sospechar migración. No es normal que el cuerpo ‘redistribuya’ un relleno de forma saludable”, dice Torres del Río.

    8) Edema (hinchazón) persistente en piernas o sensación de pesadez

    La inflamación o compromiso linfático puede producir hinchazón persistente, sensación de pesadez o cambios en la circulación. En algunos casos, la piel puede sentirse tirante o dejar marca al presionarla.

    “Cuando hay edema persistente, el sistema linfático puede estar sufriendo. No es un tema menor: el linfático es clave en defensa, drenaje y salud de tejidos”, explica.

    9) Alteraciones al caminar o al sentarse

    Si sentarse produce dolor intenso, si caminar se vuelve limitado por ardor o presión, o si hay cambios en la postura por evitar el dolor, puede existir compromiso de planos profundos o inflamación relevante.

    “El cuerpo se adapta al dolor, pero esa adaptación termina lesionando otras zonas: columna, cadera, rodillas. Por eso no hay que normalizar el sufrimiento”, advierte.

    Señales sistémicas: cuando el cuerpo entero “avisa”

    Además de lo local, algunos pacientes reportan síntomas generales que pueden estar asociados a una inflamación sostenida o respuesta inmunológica. Estos síntomas no prueban por sí solos la causa, pero son relevantes si se suman a antecedentes de inyección de biopolímeros.

    10) Fatiga persistente y sensación de “inflamación general”

    Cansancio que no se explica, baja energía crónica, sensación de malestar general o de “estar enfermo” de manera recurrente puede ocurrir cuando hay inflamación sostenida.

    “Hay pacientes que sienten que el cuerpo no les responde igual desde hace meses. Si eso coincide con dolores locales y antecedentes de biopolímeros, se debe investigar”, afirma.

    11) Brotes cutáneos, urticaria o reacciones tipo alergia recurrente

    Episodios repetidos de urticaria, enrojecimiento o picazón sin causa clara pueden ser parte de una respuesta inflamatoria o de hipersensibilidad. No siempre se relaciona, pero es una señal para valoración si coexiste con síntomas locales.

    “Las alergias se tratan, pero también se buscan causas. Cuando se vuelven recurrentes, hay que ampliar la mirada clínica”, explica.

    12) Dolor articular o muscular inexplicable

    Dolores generalizados, rigidez o sensibilidad muscular sin lesión aparente pueden aparecer en contextos inflamatorios. Su evaluación debe descartar otras causas, pero el antecedente de biopolímeros es un dato clínico importante.

    “Lo prudente es no atribuir todo a los biopolímeros sin evaluar, pero tampoco ignorarlos si hay un historial claro y síntomas compatibles”, señala.

    13) Episodios de fiebre, escalofríos o signos de infección

    La fiebre, escalofríos o empeoramiento súbito de un área inflamada puede sugerir infección y requiere atención prioritaria.

    “Cuando hay fiebre con dolor y enrojecimiento en la zona, no se espera. Se consulta”, advierte.

    Lo que NO se debe hacer ante señales de alarma

    Una de las mayores preocupaciones del Dr. Torres del Río es que, ante los síntomas, muchos pacientes caen en soluciones rápidas: inyecciones “para desinflamar”, masajes agresivos o sustancias para “disolver” el material. Estas medidas pueden empeorar migración, causar lesiones, infecciones o distorsionar tejidos.

    Evitar:

    Masajes fuertes o aparatología intensa sin diagnóstico: puede facilitar desplazamiento del material o aumentar inflamación.

    Inyecciones no indicadas (corticoides sin control, “enzimas”, sustancias desconocidas): pueden enmascarar el problema y agravar tejidos.

    Automedicación prolongada: analgésicos y antiinflamatorios pueden ocultar señales de progresión.

    Procedimientos estéticos encima del área (rellenos, calor intenso, “moldeo”): sin evaluación médica, aumentan riesgo.

    “Cuando hay señales de alarma, lo primero es diagnóstico. Tratar sin saber qué está pasando puede transformar una complicación manejable en una lesión severa”, afirma.

    Qué evalúa un especialista: el camino clínico responsable

    Un enfoque médico serio no se basa en suposiciones ni en “recetas universales”. El Dr. Torres del Río indica que la valoración debe ser ordenada:

    Historia clínica detallada: qué se aplicó (si se sabe), cuánto, dónde, hace cuánto, y cómo han evolucionado los síntomas.

    Examen físico: localización de nódulos, textura de tejidos, coloración, dolor, movilidad, zonas de calor, cicatrices, deformidades.

    Apoyo diagnóstico según el caso: imágenes y exámenes orientados a ubicar material, valorar tejidos, descartar abscesos o colecciones, y evaluar extensión del compromiso.

    Definición de prioridades: hay casos donde lo urgente es controlar infección o inflamación aguda; en otros, planificar manejo reconstructivo.

    “En biopolímeros, cada paciente es un mapa distinto. No se puede prometer un resultado sin ver la extensión y sin entender cómo ha reaccionado el cuerpo”, explica.

    Diferenciar “retirar” y “reconstruir”: una distinción clave

    Otro punto frecuente de confusión es pensar que todo se resuelve “quitando” el material. En muchos casos, además del retiro, se requiere reconstrucción de tejidos, especialmente en glúteos, donde la inflamación crónica y la fibrosis pueden deformar, endurecer o dañar el soporte.

    “Retirar es una parte. Reconstruir es otra. Hay pacientes que necesitan que se recupere la forma, el soporte y la funcionalidad de los tejidos. Eso se planifica, no se improvisa”, señala Torres del Río.

    La reconstrucción puede incluir estrategias para restaurar volumen y contorno de manera segura, corregir deformidades y mejorar la calidad de la piel y tejidos afectados, siempre con criterios de seguridad y expectativas realistas.

    Cómo se ve una complicación en el día a día: señales prácticas para el paciente

    Para el público general, la información clínica puede sonar abstracta. Por eso, el Dr. Torres del Río propone señales prácticas: “Si usted siente que algo cambió en su cuerpo y no puede explicarlo con normalidad, no lo minimice”.

    Algunas señales concretas incluyen:

    “Antes podía sentarme sin dolor; ahora no”.

    “Me apareció una bola dura donde antes no estaba”.

    “La piel se puso roja y caliente de forma recurrente”.

    “Se me inflama una pierna sin razón clara”.

    “Me sale líquido o tengo una herida que no cierra”.

    “Siento ardor constante o punzadas profundas”.

    “Esas frases suelen preceder a diagnósticos importantes. Y cuando se atienden temprano, se evitan escaladas”, explica.

    Prevención secundaria: qué puede ayudar a detectar a tiempo

    Aunque la prevención primaria es no aplicarse sustancias no autorizadas, muchas personas ya viven con biopolímeros. En esos casos, la prevención es detectar temprano y evitar prácticas que agraven.

    Recomendaciones generales de vigilancia (siempre individualizadas):

    Mantener un registro de síntomas: cuándo aparecen, qué los empeora, qué los calma.

    Evitar manipular o masajear fuerte zonas con nódulos o dolor.

    Buscar valoración médica si hay cambios progresivos, inflamación recurrente o dolor persistente.

    Consultar de inmediato ante fiebre, secreción o heridas que no cierran.

    “El mensaje no es ‘vivir con miedo’; es vivir con criterio. Si hay señales, se actúa. Si no hay señales, se vigila con calma”, puntualiza el especialista.

    Un llamado editorial: tratarlo como salud, no como vergüenza

    Las complicaciones por biopolímeros suelen venir acompañadas de silencio. Muchas personas evitan consultar por culpa, por temor al juicio o por haber sido engañadas. Para el Dr. Torres del Río, ese silencio es un riesgo: retrasa el diagnóstico y permite que la inflamación avance.

    “La persona que consulta no necesita regaños: necesita solución y acompañamiento médico. El estigma es parte del problema, porque hace que el paciente llegue tarde”, afirma.

    En esa línea, el enfoque editorial apunta a dos responsabilidades: informar sin morbo, y explicar que pedir ayuda es un acto de cuidado. La salud no debería condicionarse al miedo a ser señalado.

    Cuándo la situación es urgente

    Aunque cada caso requiere evaluación, el Dr. Torres del Río enfatiza signos que justifican atención prioritaria:

    Fiebre con dolor y enrojecimiento en la zona.

    Secreción, mal olor, heridas abiertas o fístulas.

    Dolor incapacitante de aparición súbita.

    Hinchazón importante de una pierna, cambios de coloración marcados o sensación de compromiso circulatorio.

    Empeoramiento rápido de deformidad o aumento de volumen con calor local.

    “Cuando hay signos de infección o deterioro rápido, no se espera. Es mejor consultar y descartar, que perder tiempo valioso”, advierte.

    Conclusión: señales de alarma no son para alarmar, sino para actuar

    El punto central, insiste el Dr. Juan Carlos Torres del Río, es que el cuerpo suele avisar. La migración y las complicaciones por biopolímeros rara vez aparecen “de la nada”; tienden a dar señales progresivas. La diferencia entre un manejo más sencillo y uno complejo suele ser el tiempo.

    “Si usted tiene antecedentes de biopolímeros y empieza a notar dolor persistente, inflamación recurrente, endurecimientos, cambios en la piel o síntomas que ‘se mueven’ a otras zonas, no lo normalice. Busque valoración. Las señales de alarma existen para protegerlo”, concluye.
